1. What forms do I need to complete for an address change?

The Emergency Contact Form, W-4 form, and the Residency Certification Form need to be completed when you move to a new address.

2. Where can I find Aetna "In Network" providers?

Please visit www.aetna.com or call 1-800-370-4526 for a list of network providers.

3. How do I check on my health insurance claim?

To get information on your claim, register for Aetna Navigator. There is a form on this page named "Register for Aetna Navigator" which you can use for directions on how to do this. If additional help is needed, send an email to DVHTclaims@dvit.com. DO NOT call Aetna directly.

4. What is the overall deductible for my health insurance plan?

For IN NETWORK services, the employee only deductible is $1,500 and family is $1,500 per individual or $3,000 for family.  For OUT OF NETWORK, employee only is $3,000 and family is $3,000 per individual or $6,000 for family.  Limerick Township pays the first half of the IN NETWORK deductible.

5. Is there an out of pocket limit on my healthcare expenses?

Yes.  IN NETWORK for employee only is $6,350 and family is $6,350 per individual or $12,700 for family.  OUT OF NETWORK max out of pocket is $10,000 for employee only and for family $10,000 per individual or $20,000 for family.
